POSITION TITLE:  Community Support Associate

WORKSITE:  Pulaski County Clinic

DEPARTMENT:  Clinical Services

DEGREE/CREDENTIAL:  High School/GED w/ 1 yr. experience working w/ individuals who receives services for primary mental health disorder and one of these co-occurring is orders; substance abuse, IDD, or physical health disorder.

TYPE:  Regular Full-time

SKILLS:  Works directly with case managers and assists in the delivery of case management services, monitor and coordinate consumer’s progress, and provide transport to clients to required appointments
