Position:  Kentucky Moms MATR (Maternal Assistance Towards Recovery) Case Manager

Employment Type:  Regular Full-Time

Credentials required:  Bachelor’s Degree preferably with emphasis in Behavioral Science/Human Services and 1-year relevant experience working with the designated consumer population.  Must maintain a valid driver’s license with acceptable driving record throughout employment as driving is required; use of personal vehicle and maintenance of personal car insurance is required.

Special skills:  Work with substance exposed mothers.  Travel inside and outside of the region is required.  Some in-state overnight stays required for trainings, etc.  CADC a plus.

Primary Functions:

• Maintains face-to-face in-home/community contact
• Provides assistance in the development of the consumer’s treatment plan, accessing services from other agencies and organizations for wrap around needs and make appropriate referrals and baseline screenings.
• Assist with the development of a service plan as well as a sound knowledge base of standards of professional practice and principles of their respected discipline.
• Provide services as documented in the consumer’s service plan.  Provide services within time requirements as set forth in agency policies and procedures.  Ensure the accuracy, content, and completeness of consumer’s case records.
• Provide on-going maintenance of consumer’s case records and adhere to standards to provide billable services under Managed Care Organizations.
